PATIENT HISTORY QUESTIONNAIRE -- There are three questionnaires: one for female patients, one for male patients, and one for children (male or female). Please complete one questionnaire per patient.
SFM does NOT accept any completed documents electronically. After downloading these documents, please print them, complete them, and bring them to your first office visit.
